Mission
The mission of Community Partnership on Aging is to promote independence, a healthy lifestyle, community involvement, and opportunities for personal growth for older adults, as well as support for family caregivers. This mission is carried out through planning, coordinating, and providing services that enhance the dignity, security, and overall well-being of the older adults in our communities.
 
Safe At Home
In line with the mission and goal of assisting older adults in maintaining their independence and safety in their homes, Community Partnership on Aging is launching a new program, entitled, "Safe At Home." The Safe At Home Program is a fund of both durable medical equipment and dollars that will enable low income older adults that reside in member cities to apply for and receive tools and equipment that will assist in adapting their homes for comfort and safety. The tools and equipment may include (but are not limited to): grab bars for showers, baths and other transitional areas; transition strips; motion detector lights; carpet stretching; light bulbs; flashlights; batteries; radios; shower chairs; hand-held shower faucets and more; hand rails; and other items determined to be needed. The Safe At Home Program will be administered by a Community Partnership on Aging Social Worker and will include a home assessment as well as application with income and asset verification.
 
Safe At Home will also utilize local businesses and contractors for purchase and installation of the equipment, which will, in turn, assist in boosting the local economy. Applications for the program may be accessed at the Community Partnership on Aging Website at www.communitypartnershiponaging.org, or at the agency's main office, 1370 Victory Drive, South Euclid.
